Our occupational therapists visit clients in their own homes to assess how they’re managing day-to-day tasks. We look at their environment, physical abilities, and routines to recommend ways to improve safety, independence, and quality of life.

Ideal for: Older adults, people recovering from injury, or those with disabilities

Transfer Assessment
When a client is moving between care settings, for eg. from hospital to home, we carry out a transfer assessment to ensure their new environment meets their physical and functional needs. This helps prevent setbacks and supports a smooth transition.
Key focus: Continuity of care, appropriate supports
Ideal for: Clients recently discharged from hospital or changing care environments

In-Home Care Help & Support
We work closely with clients and families to support everyday tasks like dressing, showering, or cooking. Our therapists provide practical strategies, training, and adaptive tools to help clients remain confident and independent at home.
Key focus: Functional independence, confidence, skill building
Ideal for: Clients with mobility challenges, chronic illness, or post-operative needs

Home Modification
We assess your home and recommend tailored modifications, for eg. grab rails, ramps, step-free showers, or stair lifts. Our goal is to reduce the risk of falls and make daily living safer and more comfortable.
Key focus: Accessibility, safety, falls prevention
Ideal for: Seniors, wheelchair users, or anyone at risk of injury at home
